Do I need a Meta Business Manager account?
Not for a basic ad. Meta's help pages state that a Business Manager account isn't required to create and run ads on Instagram. But the moment more than one person is involved, or you have a pixel and audiences worth protecting, you want one.

Meta describes Business Manager as the place to manage ad accounts, Pages, apps and permissions together — showing which assets belong to your brand, who on your team can access them, and which external partners such as agencies have access.
In plain terms, it answers one question: who owns what, and who can touch it.

The one mistake that really hurts
Letting someone else own your assets. If your agency created the ad account and the pixel under their business, then when you part ways you can lose the account, the pixel history and every audience built from it — and none of that can be rebuilt backwards.
Set it up the right way round: you own the business portfolio, your page, your ad account and your pixel. Partners get access, and access can be withdrawn.
