How does Google Ads actually work?
Every time somebody searches, Google runs an instant auction to decide which ads appear and in what order. You don't buy a position — you enter that auction, and both your bid and the quality of your ad decide whether you win it.

What happens when someone searches?
Google describes it as a lightning-fast ad auction that runs every time someone searches. Your ad is either eligible for that auction or it isn't, and that's decided per search — not bought in advance.
What is Ad Rank?
Ad Rank is what decides whether your ad shows at all, and where. Google says it's calculated from several factors:

So do I pay whatever I bid?
No — and this is the part most people get wrong. Google states you only pay what is minimally required to clear the Ad Rank thresholds and beat the advertiser immediately below you. Your bid is a ceiling, not a price.
Google also says higher quality ads can often lead to lower costs per click. A better ad and a better landing page genuinely make you pay less for the same position.
What is Quality Score then?
Google is clear that Quality Score is not an input in the auction. It's a diagnostic built from expected click-through rate, ad relevance and landing page experience, to show which keywords hurt the user experience. Treat it as a to-do list, not a grade.
What does this mean in practice?
